We are looking to add a part-time or PRN Medical Social Worker to our Kalamazoo team! The person we are looking for enjoys appreciation for their hard work, makes a difference providing quality care and wants to be part of a work family not just another employee.

A Medical Social Worker is responsible for the following:

· Plans/coordinates all social services within Residence.

· Participates in the development of the plan of care.

· Participates in patient care conferences.

· Maintains current knowledge of Medicare Conditions of Participation for social work services.

· Develops a cooperative relationship and communicates effectively and professionally with physicians.

· Participates in the coordination of All Seasons' services with the services of other community resources; uses community resources.

· Serves as a resource person to employees, patients, physicians and other allied healthcare providers.

· Attend meetings, CEUs on a regular basis

Requirements:

Working for a home health care agency requires the following:

• Computer skills; must be able to work with an EMR system.
• Current Michigan license in good standing.
• Reliable auto and current auto insurance
• Current CPR.
• Can work well independently.
• Solid understanding of Medicare/Medicaid guidelines as it applies to Home Health Care.
• 3-5 years of experience is preferred
• 1 year Home Health Care experience is strongly preferred.

Job Types: Part-time, PRN

Pay: From $60.00 per hour
